We're exicted about our partnership with Clover to accept credit card payments. Before we send you there though, we need a little information.

If you're on a mobile device, just scan the QR code that appears on your latest Family Counseling Center statment. These QR codes are unique to the account on the statment. If you're in a household that receives multiple statements, make sure you scan the one specifically you're wishing to make a payment for.

Without using a QR code, you can still enter your account number manually and amount you wish to pay below.
It's important you populate this form accuractly and use it for future payments as well. This helps us direct your payment made through Clover to the correct account.
After we collect that little bit of information, you can proceed to Clover to make the actual payment.
